Transition Age Youth Programs serve current and former foster and probation placement youth ages 15-24. We provide an array of programs and services including counseling, skills building, a drop-in resource center, and subsidized supportive housing. Our mission is to support and empower youth in making a healthy transition into successful adulthood.
The Counselor for the Transition Age Youth Program is responsible for establishing a close working relationship with an assigned caseload of foster, probation and after care youth to provide quality comprehensive clinical services. The Counselor will provide case management and mental health services, and act as liaison with outside agencies to assist youth in developing the necessary skills to live independently. Services will be delivered with dignity and respect for each individual client and a commitment to their right of choice.
Duties & Responsibilities:
-Establish a close working relationship with and provide quality clinical services to an assigned caseload of youth aged 15-24.
-Organize on-duty time in such a way that a percentage of the workday is spent providing Medi-Cal billable mental health services including counseling and treatment planning. Maintain clinical charts according to established guidelines.
-Assist youth with aspects of living independently for the first time (maintaining housing, employment, money management, social relationships, education, etc.)
-Facilitate workshops, field trips, and other activities for youth.
-Work closely with other agency and County staff, including Career Works, Children’s Mental Health staff, Child Welfare Services Social Workers, other counties’ ILP’s, Cabrillo College and the Housing -Authority in order to link youth to needed services.
-Facilitate payments of incentives, scholarships, housing assistance and other payments to clients.
-Maintain a valid license and insurance, and transport clients on a daily basis.
-Bilingual Spanish speaking ability strongly encouraged.
-B.A. in related field required. MA preferred. Clinical supervision available.
-At least two years supervised work experience with young adult populations.
-Ability to perform support services that assist clients in everyday living skills.
-Demonstrated ability to organize and complete tasks.
-Must be self-motivated and able to work alone, as well as accept supervision.
-Must have a valid CA driver’s license, vehicle and insurance, and agency insurance clearance.
-Fingerprinting and criminal record clearance required.
-If in recovery, two years clean and sober required.
