Supported Living Service Coordinator - Job Responsibilities

Coordinate, train, and supervise the direct support teams for clients with developmental disabilities. Oversee the operations of support teams for 6-8 people with disabilities. Interested parties should live within 20 miles of the service area, which is Santa Cruz CA, have reliable transportation and a working cell phone.

The core responsibilities of this role are listed below.

  • Safety - Assisting with training and supervising a team that work directly with our clients to maintain a clean, safe and self-directed household.
  • Personal Finance Support for Clients (As needed) - Assist with banking, rent, bills, debts, SSI, Medicare/MediCal, petty cash, spending oversight.
  • Feedback Regarding Medical Coordination - Our nurse schedules doctors' appointments and oversees medical care. You will assist with staffing coordination as needed for appointments.
  • Communication Liaison and Advocate - You`ll work alongside clients` social workers, families, staff, work or day program, and other key members of their lives to ensure seamless support.
  • Community Activities - Registering for classes, community activities, and developing more services as clients` needs change through their lives.
  • Communicating with the San Andreas Regional Center - write progress reports and submit to regional center case worker and conservator, invite staff and attend progress meetings every three months, keep contact information for workers, keep calendar of progress meetings, write special incident reports, fax in all health care reports, document all communication including date.
  • File Maintenance - Maintain all files for client, household, landlord, day program, etc.
  • Schedule Management - Overseeing the current client and staff schedules. Making sure client has staff when needed, as well as working shifts with clients. Assisting clients with planning vacations, medical surgeries/recovery, family visits, changes in jobs or day programs, and other life events.
  • Staff Supervision - Assist with hiring, supporting, training and supervising staff for clients. Familiarize yourself with employee handbook; keep a current schedule of staff; keep current staff contact information. Organize and facilitate house meetings as needed.
  • Housing Responsibilities - Maintain good relations with clients` property managers, landlords, and neighbors. Ensuring that clients secure affordable, safe housing that is well-maintained by client and the staff who support them. This includes assigning chore duties to clients and staff as needed, or providing support to Lead Staff in households with Lead Staff. Update client safety supplies, first aid, emergency kit, flashlight and safety inspections.

Required Skills/Experience:

  • Either a college degree and at least two years working with people with developmental disabilities, or five years of experience working with people with developmental disabilities, including management experience.
  • The ability to work autonomously, while still providing excellent support to various teams within the company.
  • Excellent verbal and written communication skills.
  • Solutions-oriented approach to problems, and the ability to approach challenging circumstances effectively and diplomatically.
  • Proficiency in Microsoft Office Suite and Google Workspace.
  • Supervisory experience strongly preferred.
